1/13/2024 EnchantaVaughn Approved QMHP-A inquired about Kailaâs thoughts, feelings, and emotions. QMHP-A accompanied Kaila out into the community to assist her with increasing her IADLâs. While awaiting clothes to finish QMHP-A discussed self-esteem and worked with Kaila on self-esteem and confidence building. QMHP-A modeled appropriate communication skills and provided Kaila with an opportunity to engage in the use of appropriate communication skills. QMHP-A promoted healthy expectations and hope throughout the session. QMHP-A discussed self-image and ways to improve your attitude and build confidence herself as well as brainstorm things about herself on how she can better herself as a parent or better her parenting skills. QMHP-A instructed Kaila to discuss with QMHP-A positive traits in regards to herself and asked her to identify things she could change about herself if she wanted to change something. QMHP-A processed each trait and encouraged Kaila to replace those negative thoughts with positive thoughts. QMHP-A also reviewed how tone and inappropriate body language coincided with self-esteem and confidence as well. Kaila appeared to be receptive to QMHP-Aâs interventions and services that were provided to her during the session. Kaila was able to actively participate in activity provided as she waited for her clothes to be completed. Kaila remained positive and patient as she stated that normally she would just take her clothes home and fold them but to remain somewhat organized she was going to stay and fold and separate her clothes. Kaila identified positive traits in regards to herself however she stressed the fact of her needing to gain trust in others (i.e. boyfriend and her boyfriendâs mother). Kaila agreed to work on her tone and body language because she admits it has a lot to do with her depression and why she stays so upset at times. Kaila continued to express to QMHP-A, âI am so glad services are back because there is a lot of things I cannot do on my own and need assistanceâ.
